Points to note:
1. 8 hours fasting is required. We will collect blood and urine sample.
2. Additional home visit transport fee of RM25 applies for locations with confirmed phlebotomist availability. Alternatively, you may visit another nearby location for blood collection.
3. 3-7 days advanced notice is required for appointment booking.
4. Time of collection of blood samples: 7.30am to 11.00am daily.

(3) WhatsApp your Payment Slip to 0143447226
(4) I’m aware there I will be arrange in 3-7 days for d blood collection after payment
